Cookies, Coordination, and a Path to Housing: How This Week’s Case Conferencing Took a Different Turn
This past week, case conferencing looked a little different - and the impact was undeniable.
As we navigate a year filled with uncertainty and change, our team wanted to try something more intentional and focused. Instead of our usual agenda, we decided to zoom in on the top 8 individuals who have been staying the longest in emergency shelter across our CoC. These are often the most complex cases, where progress can feel slow and housing solutions hard to find. But we believed that by rallying more intentionally around them, we might be able to move the needle.
So we took a different approach.
Ahead of the meeting, we looked up each individual’s case history and sent a personal email to every provider who’s ever worked with them. The goal was simple: bring the right people to the table, spark communication early, and show that these cases matter—not just in numbers, but in names and stories.
And it worked.
Even before the meeting started, case managers were already talking. In corners of the room, emails were being pulled up, timelines discussed, and partnerships forming. The collaboration had started before we even sat down.
Once we got to the actual case conferencing, the shift was palpable. The conversation was more engaged, more strategic, and more hopeful. Providers shared insights, challenges, and creative solutions. We didn’t just talk about barriers—we talked about next steps. Together.
Importantly, this kind of deep coordination is only possible because of the strong data sharing agreements and ongoing training that underpin our system. Every provider at the table operates under a shared commitment to confidentiality, data security, and ethical collaboration. These agreements and trainings build the trust required to share information responsibly and ensure that each individual’s privacy and dignity remain protected.
The best part? Five of those eight individuals are now on their way to housing.
It was a reminder that when we come together with clear focus and shared accountability, progress is possible—even with the most complex cases. Oh—and we brought cookies. 🍪
Because let’s be honest: it’s been a long year with a lot of unknowns, and sometimes a little sweetness helps us all feel a little less ~crumby~.
This week showed us what’s possible when we lead with intention, connection, and just a bit of cheer. We’re grateful to all the providers who showed up—not just physically, but emotionally and professionally. Let’s keep building on this momentum.
